Your First Visit

Your first appointment includes a thorough exam, any necessary imaging and a conversation about your goals. We’ll answer your questions and design a personalized plan.

New Patient Forms

• Please arrive 15 minutes early to complete paperwork
• Bring a photo ID and your insurance card
• Bring a list of any medications you take
• Note any dental concerns or questions you have

Office Hours

Monday – Friday, 9:00 AM – 5:00 PM. Weekend appointments are available by request.

What to Expect

From the moment you arrive, our multilingual team will make you feel welcome and comfortable. We speak English, Farsi, Armenian, Aramaic and Russian.
